DAY INSURANCE FORMS

(please refer to PCANT for confirmation as to how many Day Insurances any one individual is allowed each year.)

Please note a full register of any Day Participants must be kept by the club with details confirming which Day Participants participated and at what event.

Day Insurance Application Form 2016
This form must be completed by every participant who is not a PCANT Member.

Day Insurance Waiver Form 2016
This form must be completed by every participant who is not a PCANT Member.

Day Insurance Remittance Form 2016
This remittance must be completed after each event and forwarded to PCANT with payment within 30 days of each event.
